Workforce Confidentiality Agreement

I have read, fully understand, and agree to be bound by every section of the Workforce Confidentiality & HIPAA Agreement above. I have had the opportunity to ask questions and request legal review if I wished.

I will complete HIPAA Privacy, Security, and Breach Notification training within 30 days of being granted access to PHI, and annually thereafter, and I will complete any role-specific training PrescriberNow requires.

I will never paste, upload, transmit, or share any PHI with ChatGPT, Claude, Gemini, Copilot, or any other AI assistant, productivity tool, search engine, social network, or external service that is not covered by a Business Associate Agreement with PrescriberNow.

I intend my typed full legal name below to serve as my electronic signature under E-SIGN and UETA, with the same legal effect as a handwritten signature on a paper document.
